Applications can be processed for placement in a Summer Camp Session once you have completed the 3 Step Application Process:
Submit an online Summer Camp Application
Once posted, you may access the online application on the CAMP home page. In the Applications box, click on “2021 Summer Camp Application” and be sure to check your email for additional guidance through the online submissions and for printable forms that may be required.
NOTE: Please note that CAMP’s applications are now mobile-device friendly (smartphones, tablets, etc…). If you cannot print the forms, please read the Paper Application Box below for more details.
Submit Physician's Report
CAMP’s Physician’s Report form is provided during the application process.
Download and print the Physician’s Report Form then have the doctor fill it out based on a current, annual physical exam.
Upload the completed form to your camper’s application OR fax it to (210) 671-5244 OR mail to PO Box 27086, San Antonio, TX 78227, attn: Family Support.
CAMP will not accept Physician’s Report forms with an exam date older than one year for placement; a new physical exam may be required prior to attending camp if the exam is over a year prior to attending. Other physical exam forms may not be accepted.
Submit Payment for Summer CAMP Fee
Payment or agency authorization does not guarantee placement. All other required paperwork and forms must also be received.
If you prefer to mail in your check or credit card payment, please print the “Payment Authorization” form and mail it to the address listed.
All financial forms or requirements in this section are provided in the email sent after completion of Part 3 online.
CAMP’s full fee is $1700. If your Camper has agency funding OR you are unable to afford the full fee, please review the Financial Assistance box below.
You will receive a status update on your Summer CAMP Application within 10-14 business days from the receipt of online Application, Physician’s Report, and Agency Authorization or Paid Fees with corresponding forms.
Upon acceptance, you will receive an email to confirm your Camper’s placement. You will also receive a confirmation packet email approximately 2 weeks prior to your Camper’s session which will include a map, packing list, check-in time, policies, and medication information.
WAIT LIST:If you enroll for a space on a Wait List, payment/authorization is not required unless a space becomes available. You will not be able to pay online, and please do not mail a payment.
CAMP’s full fee is $1700. If your Camper has agency funding OR you are unable to afford the full fee, please review the following to see if these options may apply:
If your Camper has funding through a medicaid waiver program (MDCP, CLASS, TxHmL, HCS, DBMD) or other non-waiver agency, you must have your Case Manager submit a letter of authorization to us guaranteeing payment of the full camp fee. Authorizations should be faxed to (210) 671-5244.
If your Camper does not receive funding, you can use the Scholarship Tier payment option to reduce the full fee. You will need to submit your 2020 IRS 1040 tax form along with corresponding W-2’s to complete the program requirements.
If your household received no taxable income for 2019 then download the “Statement of Non Taxable Income.” Complete the form, and then submit it with supporting documentation and payment (unless paid online).
Foster Families – If your Camper has been placed in foster care by DFPS, then download the “Foster Parent Worksheet” and follow instructions on that form. Submit the form with payment.
Additional financial assistance is available by following directions within the application, once it becomes available.

Only request a paper application if you do not have internet access. The online application is more efficient, as paper applications take more time to complete and process. Paper applications will only be mailed to individual families, not to agencies.
In February 2021, if you do not have online/internet access, call CAMP’s administration office 210-671-5411 to request a paper application.
Complete all sections of the application thoroughly. Incomplete applications may be returned for completion, which will delay the application process for your Camper.
Include the required documents and payment (if needed based on payment option chosen) listed in steps 2 and 3 above with the application.
You will be updated regarding the status of the Summer CAMP application within 10-14 business days from the receipt of paper Application, Physician’s Report, and Agency Authorization or Paid Fees with corresponding forms. Paper Applications will not be mailed after February 26, 2021. All paper applications will need to be sent and postmarked by March 19, 2021. Paper applications should only be mailed; electronic copies (fax or email) will not be entered or reviewed.
Upon acceptance, we will send you a postcard to confirm your acceptance and placement into your Camper’s session. You will also receive a confirmation packet in the mail approximately 2 weeks prior to your Camper’s session which will include a map, packing list, check-in time, policies, and medication information.
