Summer Camp Application


Applications are processed for placement in a Summer Camp Session once you have completed the Application Process:


Submit Online Application

CAMP’s application is family-driven.
  • Log into your Parent Dashboard (or, if you don’t yet have an account, use “Create an Account”). This is where you enter an Enrollment Passcode (See Step 1) to view the Summer Camp Enrollment options available for your Camper.
  • Follow prompts at the top of each section to select, navigate, and check-out the enrollment.
  • Required supplemental forms: COVID-19 Waiver, Programs Permissions, and Medications


Submit Additional Required Forms

Required for every Camper’s Application—Upload or complete these in the Forms tab.
  • Camper Information Part 1, Part 2, and Part 3.
  • Physician’s Report CAMP’s physical form must be completed by a physician based on a physical exam within the past 12 months. Physicals are valid for one year from date of exam.
  • Immunization Record (or Affidavit) is required this summer.
Applications are processed on a first-come, first-served basis based on complete applications.


Submit Payment Option Information

Payment or agency authorization does not guarantee placement. For Campers who have Agency Funding:
  • It is your responsibility to (a) request your Camper’s respite authorization letter and (b) ensure we receive it.
  • Contact the agency responsible for your Camper’s funding. Provide them with the dates of the requested session and request the letter be emailed to
For Families using Parent Pay options:
  • If you are able to pay the Camper’s full fee, no additional information is needed.
  • If you use a qualifying option in the Scholarship Tier, you must upload your 2020 IRS 1040 (required)


CAMP reserves the right to determine final session placement once a full application is received. Completed applications will take 10 to 15 business days for processing. Family Support staff will email you with any questions, missing information, and the status of your application. If your Camper’s application is placed on the waitlist, you will receive more information about what you may expect if spaces become available. CAMP reserves the right to modify any sessions or placements deemed necessary. CAMP is committed to keeping you informed regarding any information that may affect your Camper’s enrollment. Continue to watch your email for possible updates to our policies or other information. A confirmation packet is attached in an email which is sent 2 weeks prior to the session. It will contain packing instructions, directions, check-in process, etc…

Additional Financial Assistance

Assistance is available to families whose Camper does not have funding via an agency and the Camper family is unable to afford the Scholarship Tier that they qualify for. Email to request more information!

Paper Applications

CAMP’s applications are now mobile-device friendly (smartphones, tablets, etc…)! If you cannot print the forms needed, please call to request them and we will mail them to you. Only request a paper application if you do not have internet access. The online application is more efficient, as paper applications take more time to complete and process. Paper applications will only be mailed to individual families, not to agencies.
  • In February 2021, if you do not have online/internet access, call CAMP’s administration office 210-671-5411 to request a paper application.
  • Complete all sections of the application thoroughly. Incomplete applications may be returned for completion, which will delay the application process for your Camper.
  • Include the required documents and payment (if needed based on payment option chosen) listed in steps 2 and 3 above with the application.
  • You will be updated regarding the status of the Summer CAMP application within 10-15 business days from the receipt of paper Application, Physician’s Report, and Agency Authorization or Paid Fees with corresponding forms. Paper Applications will not be mailed after March 5, 2021. All completed paper applications will need to be postmarked for return no later than April 2, 2021. Paper applications should only be mailed; electronic copies (fax or email) will not be entered or reviewed.
Upon acceptance, we will send you a postcard to confirm your acceptance and placement into your Camper’s session. You will also receive a confirmation packet in the mail approximately 2 weeks prior to your Camper’s session which will include a map, packing list, check-in time, policies, and medication information.
