School-Year Programs Application

Submit Online Application

CAMP’s application is family-driven.
  • Log into your Parent Dashboard (or, if you don’t yet have an account, use “Create an Account”).
  • Follow prompts at the top of each section to select, navigate, and check-out the enrollment.
  • Required supplemental forms: COVID-19 Waiver, CAMP Programs Permissions, and Medications


Submit Additional Required Forms

Required for every Camper’s Application—Upload or complete these in the Forms tab.
  • Camper Information Part 1, Part 2, and Part 3.
  • Physician’s Report CAMP’s physical form must be completed by a physician based on a physical exam within the past 12 months. Physicals are valid for one year from date of exam.
  • Immunization Record (or Affidavit) is required this summer.
  • All Campers with Asthma must provide their most recent Asthma Action Plan.


Payment Option Information

For Campers who have Agency Funding:
  • Application fees are waived for all Campers who have Medicaid Waiver Program / respite funds specifically for billable (overnight camp) events.
  • Respite Camp authorizations (if desired) are not to be requested until you have the opportunity to enroll for one or more camps.
  • More information about authorization requirements will be provided during the Respite Weekends and Spring Break Camps enrollments.
For Campers using Parent Pay options:
  • A $60 application fee (+$10 for each additional Camper within a family unit) is due and will be charged when the application is reviewed and approved.
  • Payment for Respite Weekend and Spring Break Camps (if desired) or other program event fees are not collected until you have the opportunity to enroll and be placed in the event.


CAMP reserves the right to determine final eligibility once a full application is received. Completed applications may take up to 7 business days for processing. Family Support staff will email you with any questions, missing information, and the status of your application. If your Camper’s application is placed on the waitlist, you will receive more information about what you may expect if spaces become available. CAMP reserves the right to modify any sessions or placements deemed necessary. CAMP is committed to keeping you informed regarding any information that may affect your Camper’s enrollment.
